What you can expect from your first appointment

Our first appointment will take an hour.  We need this time to get to know each other. We will have a conversation about your lifestyle, what health concerns you have, and what you might want to work on. We will also cover your preferences and needs as they relate to making changes. This appointment is for you and I am here to help you with your needs. 

I provide what is known as Medical Nutrition Therapy (MNT) to help people improve and manage their health.  Insurance companies may cover MNT provided by a Registered Dietitian. In order to provide nutrition counseling, I need to have certain information about your health. Please fill out the forms below and bring them to our first appointment. If you need to have the forms mailed to you, please contact me.

Forms

Physician Referral Medicare requires a referral from an MD or DO for Diabetes and Chronic Kidney Disease nutrition counseling.
